There are important health requirements that are needed as part of enrollment, and throughout the student’s scholastic career, which are based upon New York State’s Educational Laws. Please review the information below and contact us if you have any questions.

Health Examinations (physicals): Health certificates are required to be submitted to school within 30 days of the start of the school year [Education Law §903, Commissioner’s regulation §136.3 (c)(1]]. New York State law requires a health examination for:

We are also required to request a dental health certificate at the same time a grade-level health examination is required. Please note, this is not a requirement for school attendance.

Immunizations (shots/vaccines): New York State law requires all students entering or attending any New York State school (public, nonpublic, and charter schools) to have received, or have a validated, written, appointment date to receive, all required doses of immunizations for their grade level no more than fourteen (14) days after the beginning of the school year in order to attend, or continue to attend school. The immunization requirements for each grade level are outlined on NYSDOH Immunization Requirements for School Entrance/Attendance. Students who do not have the required immunizations, or have a validated, written appointment to receive required immunization, may not attend school.

Prescribed & Over-The-Counter Medications: Medications need to be provided in the home by a parent. However, when this isn’t possible, and your child needs to take medication(s) during the school day, we are required to have the following:

Based upon New York State regulations, the above requirements and forms for medication administration are required with each new school year. Orders from the previous year cannot be used the following school year. All medications and supplies must be picked up by the end of each school year or they will be disposed of. Items cannot be stored or housed at the school from school year to school year.
