Please review the information below and contact us if you have any questions.<\/p>\n        <p class=\"u-align-left u-text u-text-4\"><span style=\"font-size: 1.25rem; font-weight: 700;\"> Health Examinations (physicals): <\/span>Health certificates are required to be submitted to school within 30 days of the start of the school year [Education Law \u00a7903, Commissioner\u2019s regulation \u00a7136.3 (c)(1]]. New York State law requires a health examination for:<br>\n        <\/p>\n        <ul class=\"u-align-left u-text u-text-5\">\n          <li>A\u200bll new entrants and students in grades Pre-K or K, 1, 3, 5, 7, 9 &amp; 11;<\/li>\n          <li>Every year for students participating in athletics (sports);<\/li>\n          <li>For working papers as needed; or<\/li>\n          <li>When required by the Committee on Special Education (CSE) or Committee on Pre-School Special Education (CPSE).<\/li>\n        <\/ul>\n        <p class=\"u-align-left u-text u-text-6\"> We are also required to request a dental health certificate at the same time a grade-level health examination is required. Please note, this is not a requirement for school attendance.<\/p>\n        <p class=\"u-align-left u-text u-text-7\"><span style=\"font-size: 1.25rem; font-weight: 700;\"> Immunizations (shots\/vaccines): <\/span>New York State law requires all students entering or attending any New York State school (public, nonpublic, and charter schools) to have received, or have a validated, written, appointment date to receive, all required doses of immunizations for their grade level no more than fourteen (14) days after the beginning of the school year in order to attend, or continue to attend school. The immunization requirements for each grade level are outlined on <a href=\"https:\/\/www.health.ny.gov\/prevention\/immunization\/schools\/school_vaccines\/\" class=\"u-active-none u-border-none u-btn u-button-link u-button-style u-hover-none u-none u-text-palette-1-base u-btn-2\" target=\"_blank\">NYSDOH Immunization Requirements for School Entrance\/Attendance.<\/a> Students who do not have the required immunizations, or have a validated, written appointment to receive required immunization, may not attend school.\n        <\/p>\n        <p class=\"u-align-left u-text u-text-8\"><span style=\"font-size: 1.25rem; font-weight: 700;\"> Prescribed &amp; Over-The-Counter Medications: <\/span>Medications need to be provided in the home by a parent. However, when this isn\u2019t possible, and your child needs to take medication(s) during the school day, we are required to have the following:\n        <\/p>\n        <ul class=\"u-align-left u-text u-text-9\">\n          <li> A written healthcare provider order, (attestation is also required for independent students);<\/li>\n          <li>Written parent\/guardian consent; and<\/li>\n          <li>Any and all medications must be in their original, labeled, prescription, or over-the-counter bottles\/packaging. Any special supplies or equipment to administer the medication must also be provided to the school.<\/li>\n        <\/ul>\n        <p class=\"u-align-left u-text u-text-10\"> Based upon New York State regulations, the above requirements and forms for medication administration are required with each new school year. Orders from the previous year cannot be used the following school year. All medications and supplies must be picked up by the end of each school year or they will be disposed of.
